The Lead Licensed Social Worker provides advanced clinical leadership, direct client care, and case management oversight within the program. This role combines hands-on client support with team leadership, quality assurance, and collaboration across disciplines to ensure high-quality, trauma-informed, and outcomes-driven services. The Lead Social Worker serves as a subject-matter expert, mentor, and resource for staff while maintaining a manageable caseload.
Essential Responsibilities:
Clinical Leadership & Supervision
- Provide clinical guidance and consultation to social work staff and case managers.
- Support onboarding, training, and mentorship of new team members.
- Review case plans and documentation for quality, accuracy, and compliance.
- Model ethical and professional social work standards and boundaries.
- Assist leadership in developing and refining program practices and protocols.
Client Support & Case Management
- Conduct biopsychosocial assessments and develop individualized, goal-oriented case plans.
- Provide direct case management, crisis intervention, and stabilization services.
- Monitor client progress and adjust service plans as needed.
- Manage a reduced caseload of higher-acuity or complex cases.
Collaboration
- Partner with intake specialists and multidisciplinary teams to ensure holistic care.
- Participate in case conferences and interdisciplinary care planning.
- Coordinate care with external providers and community partners.
Resource Coordination & Advocacy
- Connect clients to housing, healthcare, employment, legal, and public benefits resources.
- Advocate for client access to services and removal of systemic barriers.
- Build and maintain relationships with community agencies.
Documentation & Reporting
- Maintain timely, accurate, and compliant client records.
- Track outcomes and contribute to program reporting and data collection.
- Ensure documentation meets licensing, funding, and organizational standards.
Program Facilitation
- Facilitate psycho educational groups and workshops on life skills, housing navigation, and job readiness.
- Support development of curriculum and group content.
Client Empowerment
- Build rapport and foster a strengths-based, trauma-informed environment.
- Encourage client self-sufficiency, goal setting, and accountability.
Shelter/Program Operations Support
- Support a safe, structured, and welcoming environment for clients.
- Assist in de-escalation and behavioral support when needed.
- Collaborate with program staff to maintain consistent client expectations.
Professional Development
- Stay current on evidence-based practices, ethics, and regulatory requirements.
- Participate in ongoing trainings and professional development.
