Creative Consulting Services, Inc. is seeking a compassionate, organized, and mission-driven Support Coordinator to join our team.
In this role, you will support individuals with developmental disabilities in living meaningful, self-directed lives while ensuring their health, safety, and overall quality of life.

What You’ll Do Advocacy & Support- Promote individual choice while ensuring safety and well-being
- Advocate for the needs of individuals served
- Support the development of natural and community supports
- Communicate the mission and values of Creative Consulting Services
- Partner with families, providers, physicians, and community resources
- Facilitate Individual Service Plan (ISP) meetings
- Coordinate services to support positive outcomes
- Complete documentation accurately and on time, including:
- Consumer contacts
- Incident reporting
- ISP, DMA‑7, and SIS documentation
- Maintain Medicaid eligibility coordination
- Stay current with state standards and CCS policies
- Manage a caseload of up to 40 individuals
- Monitor voicemail and email daily and respond within 24 hours
- Participate in required trainings and professional development
- Represent CCS professionally in the community
- Perform additional duties as assigned
- Bachelor’s degree in Social Work, Psychology, or related field Required
- At least 1 year of experience working with individuals with developmental or intellectual disabilities
- Strong organizational and communication skills
- Ability to manage multiple priorities
- Comfort using technology including Microsoft Word, Excel, and internet tools
- Passion for advocacy and person-centered services
