Overview
The Aftercare Coordinator works with graduates and program participants post attendance from the Camp Hope Program. This role provides aftercare planning and alumni services for veterans and their families. It coordinates an assigned caseload of combat veterans diagnosed with PTSD who have graduated or attended the Camp Hope Program.
Responsibilities- Foster relationships with veterans and their families who have attended the Camp Hope Program and may need additional services to access community resources.
- Attend appointments with clients who require additional support.
- Coordinate contact and continuation of care plans for aftercare clients.
- Engage and enroll veterans in need of support while addressing the unique needs of aftercare clients.
- Identify and establish relationships with community support networks.
- Provide needed services through referrals and follow-up on progress.
- Maintain professional relationships with other service providers.
- Demonstrate knowledge of PTSD, general mental disorders, psychosocial aspects of PTSD, and the needs of former residents and alumni.
- Coordinate with local authorities to conduct health and wellness checks.
- Complete additional tasks/reports as requested by the supervisor and meet productivity standards as outlined in operational guidelines.
- Collaborate with Camp Hope program staff to establish an aftercare plan for residents in the last phase of their program.
- Monitor client progress toward treatment goals.
- Must be able to work in a fast-paced environment.
- Energy and interest in follow-up calls, case finding, and outreach.
- Ability and willingness to travel.
- Able to work collaboratively in a team environment.
- Experience working with substance abuse, homelessness, veterans, and groups.
- Excellent written and oral communication skills.
Salary range: $15.00 to $20.00 hourly. We offer comprehensive benefits including medical, dental, prescription, vision, life insurance, short- and long-term disability, and generous paid vacations and holidays.
