Monday-Friday: 8:30 AM - 4:30 PM

Grade 16 - Non-Exempt

Pay Range: $22.50 - $32.02 per hour

JOB SUMMARY:

Oswego County Opportunities (OCO) is seeking a dedicated and dynamic CarePoint Program Manager to lead and support services provided through New York State's 1115 Waiver Program. This is an exciting opportunity for an experienced human services professional who is passionate about improving access to care, supporting community health, and developing high-performing teams.

In this role, you will supervise intake and care management staff, provide program leadership and oversight, build strong community partnerships, and directly support individuals in accessing critical services and resources.

OVERVIEW OF

JOB DUTIES AND RESPONSIBILITIES:

As the CarePoint Program Manager, you will:

  • Lead and supervise intake and care management staff, providing coaching, mentoring, and professional development opportunities.
  • Monitor caseloads, service delivery, and social care plans to ensure quality services and regulatory compliance.
  • Ensure program services are delivered in accordance with NYS 1115 Waiver Program standards, Medicaid requirements, and agency policies.
  • Maintain oversight of documentation, case records, and required databases to support program integrity and reporting.
  • Build and maintain strong partnerships with community organizations, healthcare providers, and internal agency programs.
  • Conduct outreach and education activities to promote awareness of program services.
  • Carry a small caseload and provide direct support to individuals seeking services through the CarePoint program.
  • Serve as an invoice administrator and assist with additional program administration responsibilities.

JOB REQUIREMENTS:
Successful candidates will possess:

  • Strong leadership and supervisory skills.
  • Excellent organizational, communication, and problem-solving abilities.
  • Knowledge of community resources and social service systems.
  • The ability to work effectively with diverse populations in a respectful, supportive, and non-judgmental manner.
  • Strong computer skills, including experience with Microsoft Office applications, databases, and electronic documentation systems.
  • A commitment to maintaining confidentiality, professionalism, and high ethical standards.
  • The ability to learn and apply NYS 1115 Waiver Program policies, procedures, and regulations.

MINIMUM QUALIFICATIONS:

  • Bachelor's degree and three (3) years of related experience
    ; and
  • One (1) to two (2) years of supervisory experience
    ; and
  • Thorough knowledge of community resources; or
  • An equivalent combination of education, training, and experience
