POSITION SUMMARY:

Person in this position will be responsible for providing support, outreach, and engagement services to veterans and service members at risk of suicide. Utilizing lived experience and trauma-informed care, the specialist builds trust with participants, connects them to essential services, and promotes hope, recovery, and wellness. Essential functions include those listed below.

ESSENTIAL JOB FUNCTIONS:

Conduct community outreach and engagement activities to identify and connect with at-risk veterans and service members.

Provide support and mentorship using shared lived experience to foster trust and empowerment.

Assist individuals in navigating mental health and substance use services, housing resources, crisis intervention, and other support systems.

Collaborate with local VA facilities, community providers, and the broader support network to coordinate services and referrals.

Participate in suicide prevention trainings and maintain knowledge of best practices related to veteran care.

Maintain accurate and timely documentation of all interactions and services provided, in compliance with grant requirements and confidentiality standards.

Support follow-up efforts with program participants to ensure continued engagement and service connection.

Promote protective factors such as connectedness, resilience, and purpose among participants.

Attend and participate in team meetings, supervision, and relevant community events.

Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of West Care Foundation; and Perform all other duties as assigned.

ESSENTIAL

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform the duties and responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

Experience and

Competencies:

Strong interpersonal, engagement, advocacy, and relationship-building skills.

Ability to organize and prioritize a caseload, coordinate multiple services, and complete documentation timely.

Ability to communicate effectively with participants, families, providers, community partners, and multidisciplinary teams.

Sound judgment, problem-solving ability, and the ability to respond appropriately to urgent participant needs.

Basic computer proficiency and ability to use electronic records, databases, email, and standard office applications.

Excellent communication skills;

Ability to exercise good judgment and discretion;

Ability to work well in a team environment;

Military/Veteran status required.

Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality;

Professional appearance and demeanor;

Valid driver’s license and ability to obtain and maintain West Care’s vehicle insurance is required; and Ability to obtain and maintain a criminal history background check.

During your tenure with West Care there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that West Care may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.

EDUCATION and/or

EXPERIENCE:

Minimum of two (2) years of experience of relevant experience.

Experience working with high-risk populations, including individuals experiencing mental health and/or substance use disorders.

Knowledge of suicide prevention principles, trauma-informed care, community resources, and challenges affecting veterans and service members.

LICENSURE/CERTIFICATION:

Not applicable for this position.

WORKING CONDITIONS:

Work is primarily performed in an office setting.

Regular travel throughout the assigned service area is required to support outreach, community partnerships, participant services, meetings, and program activities.

Travel between West Care facilities, VA facilities, community partner locations, and outreach sites may be required.

Position requires regular interaction with Veterans and individuals who may be experiencing behavioral health concerns, emotional distress, significant psychosocial stressors, or suicide risk.

Some evening or weekend work may be required to support outreach events, community activities, or program needs.
