MISSION/VISION Healthcare Alternative Systems (H.A.S.) provides a continuum of multicultural and bilingual (English/Spanish) behavioral care and social services that empower individuals, families, and communities. H.A.S. seeks to implement sustainable programs that benefit society by improving the well-being and recovery efforts of those individuals, families, and communities impacted by behavioral health problems. With eleven locations across the Illinois, H.A.S. provides services and resources to individuals at every stage in their behavioral/recovery journey so that they can be successful and become contributing members of their communities.

JOB SUMMARY Under the supervision of the Program Supervisor, the Case Manager is responsible for managing a caseload of justice-involved participants, linking participants to needed community resources, assisting in the ongoing development of individualized service and treatment plans, monitoring clinical engagement of treatment, and ensuring wraparound reentry and recovery activities and responsibilities are completed by participants.

ESSENTIAL DUTIES Researches and locates community and reentry resources for all participants, including legal, medical, housing, transportation, mental health, and financial resources.

Assists in the ongoing development of individualized, person-centered service and treatment plans that include goals around treatment, primary health, and life goals.

Monitors the engagement of the clinical treatment of the participant with the multidisciplinary team.

Links participants to needed and available community resources and provides warm handoffs to wraparound reentry services, including housing, workforce development, transportation, and pretrial/legal support.

Independently manages their caseload and documents all services provided in SAMMS and/or additional trackers as needed.

Performs contingency management planning and utilization.

Provides face-to-face reviews with the participant and/or relevant staff on a regular basis, reviewing progress, identifying barriers and practical solutions, and updating plans and goals as needed.

Attends and participates in assigned meetings.

Participates in team meetings, agency meetings and agency fundraising efforts.

Documents all service contacts on a timely basis, including face-to-face interviews, collateral and networking contacts, and correspondence, and maintains case records in accordance with agency and regulatory standards and requirements.

Assist participants in obtaining Medicaid, SNAP and other government benefits.

Collaborates with the treatment team to ensure compliance and work toward improved participant outcomes.

Provides anti-stigma education to the community and to individuals dealing with mental illness and/or substance use disorders, as well as information on prevention, treatment, and recovery.

Provides referrals and education related to job readiness, personal finance, GED and other educational programs, and systems navigation.

Takes direction, is prompt, takes initiative, completes tasks as assigned, and is able to work effectively as part of a team that includes community stakeholders.

Articulates effectively and has good communication skills.

Agrees to adhere to the Code of Ethics.

Performs other duties as assigned.

REQUIRED SKILLS/ABILITIES Has clinical and training experience in substance abuse and familiarity with the managed care environment, as well as an understanding of case management, community resources, and reentry service and treatment planning.

Good oral and written communication skills.

Commitment to ongoing professional development and training.

Willingness to work in the community and in the office.

Bilingual (English/Spanish) preferred, with a thorough understanding of the social and cultural patterns of Hispanics and other minorities.

Flexible to adapt to schedules and workload to the needs and demands of the program.

Possession of a valid Illinois motor vehicle operator’s license and willingness to use personal vehicle in course of employment.

H.A.S. requires all staff to work onsite.
