Position: Bilingual Patient Continuity and Needs Advocate

Chen Med is seeking a Bilingual Patient Continuity and Needs Advocate to support seniors with complex chronic conditions. In this role, you coordinate appointments, outreach, and follow-up to ensure seamless, high-touch care. You identify and address social and care barriers, connect patients to community resources, and reinforce care plans in both English and another language. Partnering closely with clinicians, you help reduce hospitalizations, improve adherence, and elevate patient experience while advancing Chen Med's mission to serve underserved Medicare-eligible seniors.

Responsibilities

  • Serve as primary advocate for senior patients, ensuring continuity of care across visits and providers
  • Conduct outreach calls to patients to coordinate appointments, follow-ups, and preventive screenings
  • Identify social, financial, and access barriers and connect patients with appropriate community and clinic resources
  • Collaborate with physicians, nurses, and care teams to support care plans and close care gaps
  • Educate patients and caregivers on care instructions, benefits, and clinic processes in English and another language
  • Document patient interactions, needs, and outcomes accurately in electronic health records or tracking tools
  • Monitor high-risk or non-engaged patients and escalate concerns to clinical staff as needed
  • Support care coordination during care transitions, such as hospital discharge or specialist referrals
  • Participate in quality improvement efforts to enhance patient experience and health outcomes
  • Maintain strict confidentiality and comply with HIPAA and organizational policies
