LOCATION:
CHOICE CHICAGO RIDGE - CHICAGO RIDGE, IL
Position: Full-Time 40 Hours per week
Salary: Based on experience (Range) $27.23 – 28.23 per hour.
ScheduleMonday to Friday 7:30am – 4pm
QUALIFICATIONS:- 1. Bachelor’s degree in human services field (psychology, special ed., etc.)
- 2. One-year experience with individuals with developmental disabilities
- Good working knowledge of behavior modification principles and techniques as they relate to the individual with developmental disabilities
- Current, valid Illinois driver’s license with a clear driving record
- Clear criminal background and HCWBC Registry reports, or approved waiver from DHS, QIDP certification or completion of QIDP training within 120 days of employment.

- Ensure Case Coordination for Participant
- Development of annual goals and objectives.
- Methodologies / Objectives are measurable
- Completion dates / Revisions / Changes when necessary
- Maintain participant case files
- In conjunction with the Program Manager ensure that all participants are obtaining active treatment and that their Plan is implemented.
- Train DSP staff on implementation of each participant’s annual plan and provide tools to preform active treatment.
- Work with various departments to ensure continuity of programming efforts.
