About the RoleThe Family Self-Sufficiency (FSS) Coordinator works directly with residents enrolled in the HUD-funded FSS program across Five T Management’s portfolio. This position helps each participant set meaningful one-year and five-year goals, connects them with supportive services, manages their escrow accounts, and tracks their progress from enrollment through graduation.
This role is primarily office-based with regular travel to properties across the Chicagoland area. Evening and weekend availability is occasionally required for resident events and community outreach. The ideal candidate combines strong case management skills with a genuine passion for economic mobility, and brings the discipline to keep compliance, documentation, and reporting consistently audit-ready.
Key Responsibilities Resource Networking and Community Partnerships- Develop and lead the Program Coordinating Committee (PCC); host quarterly meetings to review program progress and gather community input.
- Attend interagency meetings and maintain an up-to-date network of community resources and referral partners.
- Connect residents with appropriate agencies and follow up to confirm successful referral outcomes.
- Identify and promote local classes, workshops, and community events that support resident goal attainment.
- Represent Five T Management at community events, speak on panels, and serve on committees as appropriate.
- Develop and distribute program promotional materials; conduct outreach to drive enrollment across eligible properties.
- Review applications, schedule and conduct eligibility interviews, and notify applicants of acceptance or denial.
- Execute Contracts of Participation with new enrollees; establish one-year and five-year goals with each participant.
- Prepare welcome packets and onboarding materials for new residents entering the program.
- Assist with data entry and event coordination when housing lists open.
- Share best practices with FSS coordinators across the broader network.
- Meet regularly with participants to discuss goal progress, address challenges, and document case notes.
- Provide and document referrals for social services including employment, education, job training, childcare, and financial empowerment resources.
- Conduct occasional home visits to support participation and assess needs.
- Coordinate and facilitate life skills development classes; produce and distribute a monthly calendar of offerings and track attendance.
- Plan graduation ceremonies for program completers and produce press releases to celebrate resident success stories.
- Post and reconcile monthly escrow accruals to participant accounts; confirm accuracy and update tracking reports.
