Overview

Health Plus Staffing is seeking a Nurse Case Manager in Chicago, IL. The Nurse Case Manager will provide help to high risk infants and children getting health care and developmental services.

The Nurse Case Manager will be responsible for maintaining a caseload of approximately 100 active clients by conducting outreach activities and utilizing the referral system from High Risk Infant follow Up Program from the State of Illinois.

• Maintain a caseload of approximately 100 active clients by conducting outreach activities and utilizing the referral system from the High Risk Infant Follow Up Program from the State of Illinois.

• Bachelor Degree in Nursing from an Accredited College or University
• Minimum of 2 years' experience working in Community Health or Maternal Child Health nursing
• Active nursing license in the state of Illinois
• Education:
  
  Bachelor's (Preferred)
• Experience:
  
  Nursing: 1 year (Preferred)

• Compensation:
  Commensurate on experience
