Overview

Responsibilities- Provide counseling services at Austin Health Center with caseloads structured and managed to support effective, patient-centered care.
- Complete an individualized treatment plan for each patient on caseload, addressing all needs identified in the psychosocial assessment.
- Provide group and individual sessions to patients in accordance with their treatment plan problems, goals, and objectives.
- Review and evaluate patients’ progress and continued stay in treatment in relation to measurable goals and objectives described in treatment and care plans. Modify treatment plans to comply with changes in patient status.
- Plan and assist with the implementation of aftercare and continuing recovery plans once patients are discharged from treatment programs.
- Maintain a minimum of 50% of time in direct, face-to-face counseling contact.
- Schedule: Monday - Friday (6:00a - 2:30p)
- Start Date: ASAP
- Work Location: 4800 W. Chicago Ave. Chicago
- Pay Rate: $42,000 - $47,500; based on experience and certifications

Experience and SkillsEducation and/or
Experience:
High School Diploma or equivalent required. Bachelor’s degree (or higher) and prior substance abuse treatment experience preferred. Will consider relevant internship experience in lieu of paid employment.
Certification, Licensure, Registrations: Licensure (LPC, LCPC, LSW, LCSW) or CADC certification strongly preferred, CADC required within up to two years of hire date, dependent on experience.
