Salary range: $16.50 - $20.63 per hour. Placement within the range depends on relevant work experience and internal equity. For positions represented by a labor union, placement within the salary range is guided by the collective bargaining agreement.

Responsibilities
  • Demonstrates the four NM values of teamwork, patients first, integrity, and excellence in daily work and interactions.

  • Provides accurate information and timely updates to patients and customers. Addresses questions and concerns promptly or identifies the appropriate person and resources. Provides directions to help patients and customers find their way.

  • Maintains primary responsibility over patient plan of care through timely and consistent collaboration with treating PT/PTA(s).
  • Demonstrates teamwork by helping co-workers within and across departments. Communicates effectively with others, respects diverse opinions and styles, and acknowledges the assistance and contributions of others.
  • Uses organizational and department resources efficiently.
  • Manages work schedule efficiently, completing tasks and assignments on time.
  • Contributes to opportunities and processes for continuous improvement.
  • Participates in efforts to reduce costs, streamline work processes, improve and grow services we provide.
  • Demonstrates creative problem solving and adaptability in treating special circumstances, as evidenced by team feedback and documentation.
  • Performs appropriate evaluations and selects treatment modalities, equipment, and protocols. Conducts re-evaluations per department guidelines and adjusts the treatment plan accordingly.
  • Maintains responsibility over the course of physical therapist patient management from initial contact to discharge for functional progress, safety, and treatment outcomes. Collaborates with treating PTA(s).
  • Adheres to department policy regarding evaluations, progress notes, and discharge summaries. All notes are documented in the electronic medical record (EMR).
  • Completes daily notes reflecting patient response to treatment, performance, and status relevant to the treatment plan and discharge needs, meeting documentation requirements.
  • Provides educational materials appropriate to the patient’s level of understanding.
  • Participates in committees, department processes and special projects according to clinical expertise and performance plan.
  • Develops professional relationships with healthcare team members and participates in internal and external marketing per department expectations.
  • Maintains a culture of respectful communication and accepts constructive feedback for timely improvement.
  • Completes non-patient care responsibilities within established timelines (coverage notes, schedule changes, etc.).
  • Contributes to department/clinic educational efforts (e.g., in-services, journal clubs, orientations).
  • Acts as a clinical instructor, mentor, or observation host for student programs as per department expectations.
  • Maintains productivity standards and flexibility to meet patient care needs and department meetings outside normal hours.
Qualifications

Required:

  • Bachelor’s Degree in Physical Therapy.
  • Licensed Physical Therapist in the State of Illinois.
  • BLS from the American Heart Association at hire.

Preferred:

  • Master’s or Doctoral Degree in Physical Therapy.
