Mental Health Specialist-Assertive Community Treatment (ACT)

As a Mental Health Specialist, you will work field‑based in clients’ homes and communities to reduce hospitalizations, stabilize lives, support housing and recovery, and build independence. You will collaborate closely with nurses, employment specialists, peer support workers, and other team members to deliver 360° care.

• Deliver life‑changing care by providing substance use counseling and mental health support in clients’ homes and communities.
• Conduct thoughtful clinical assessments that shape personalized recovery goals and ignite hope.
• Drive measurable outcomes by stabilizing lives, reducing hospitalizations, supporting housing success, and preventing crises.
• Collaborate with the ACT team to build integrated support for each client.
• Assist clients in accessing housing, benefits, medical services, and other critical resources.
• Build lasting life skills and guide clients toward long‑term recovery and community reintegration.
• Act as a trusted anchor in crisis, coordinating care transitions and supporting clients during critical moments.
• Document progress and outcomes clearly and timely in the client record.

• Bachelor’s degree in social work, psychology, or a related field (Master’s preferred).
• CADC or substance use certification (or in progress) that meets MHP criteria.
• Strong communication and critical‑thinking skills with a passion for recovery.
• Valid driver’s license and willingness to transport clients in a personal vehicle.
• Ability to pass a background check.

• Base Salary: $50,000 per year + additional quarterly productivity bonuses.
• Paid Time Off: 11 vacation days, 12 paid holidays, 3 sick days, 3 personal days.
• Illinois Paid Leave: 40 hours (accrued).
