Under the direct supervision of the Director of Family Support Program (FSD) the Triple P trainer provides in home sessions using Triple P curriculum, which will include parent-skills training and psychoeducation. Providing 8-10 in home sessions as defined in the model. Maintain client records and confidential information, conduct assessments, planning, coordination and advocacy services for clients who need multiple services and require assistance in gaining access to and in using behavioral health, physical health, social, vocational, educational, housing, public income entitlements and other community services to assist the individual in the community.

May also include identifying and investigating available resources, explaining options to the client, and linking the client with necessary resources. Complete monthly, and quarterly reports. Work with other service providers, build connections within communities, other job duties as assigned.

Reports To: Director of Family Support Program

  • Provides in home sessions using Triple P curriculum, which will include parent-skills training and psychoeducation. Providing 8-10 in home sessions as defined in the model.
  • Conducts assessment, planning, coordination and advocacy services for clients who need multiple services and require assistance in gaining access to and in using behavioral health, physical health, social, vocational, educational, housing, public income entitlements and other community services to assist the individual in the community. May also include identifying and investigating available resources, explaining options to the client, and linking the client with necessary resources.
  • Outreach and engagement activities that are specific to an identified client or family and provided prior to the development of a treatment plan.
  • Attend and testify at court hearings upon written request.
  • Maintains necessary records on client services, and documenting case notes in a timely manner. Completes monthly statistics and reports as required and prepares summaries for consultation, and monitoring for referral purposes.
  • Performs other duties as assigned within the Triple P Program.
