Pay Range: $53.73 - $84.63 per hour

THERE ARE 2 POSITIONS, THIS POSITION, WHICH IS AT OUR NORTH / HARLEM LOCATION AND A POSTION AT THE MAIN CAMPUS ON HARRISON IN CHICAGO.
Summary:The scope of practice for the Ambulatory Clinical Pharmacist Specialist in primary care focuses on chronic disease management. The pharmacist will be responsible for providing patient care according to health system procedures and provider-pharmacist collaborative agreements. Additionally, the pharmacist will be expected to contribute to and participate in pharmacy department initiatives pertaining to their clinical role. The pharmacist will exemplify the Rush mission, vision, and values while acting in accordance with Rush policies and procedures.
Responsibilities:- Conduct scheduled in-person and telehealth appointments with patients to address healthcare and medication needs
- Develop and implement chronic disease management standards for clinical pharmacy staff in the ambulatory setting
- Serve as a liaison between the pharmacy department and other clinic staff as needed
- Participate in departmental and health system committee and workgroup meetings
- Clarify prescriber orders, when necessary, through direct and timely contact with the prescriber or designated practitioner
- Provide patient-specific pharmacotherapy recommendations and drug information to medical and nursing staff
- Participate in the pharmacy department's current documentation methods for clinical interventions
- Implement all drug use policies and formulary decisions of the Pharmacy and Therapeutics Committee
- Participate in safety event (ADR and medication error) reporting programs
- Precept pharmacy students and residents (PGY1 and PGY2) in assigned clinical areas
- Assess and evaluate medical and drug literature to compare and contrast therapeutic regimens and uses for new or existing drug treatments
- Implement medication safety initiatives as determined by current departmental needs and ongoing projects
- Develop operational and clinical projects as determined by current departmental needs and ongoing projects
- Serve as a liaison with other departments for collaborative research activities
- Evaluate and monitor drug therapy relative to the comprehensive needs of ambulatory care patients in collaboration with physicians, nurses, and other healthcare professionals
- Actively participate in professional organizations
Job Qualifications:
- Bachelor of Science in Pharmacy.
- Illinois Pharmacist License.
- Minimum of four years of Clinical Pharmacy experience OR PGY 1 residency completed within the last seven years, which includes Clinical / Specialty Pharmacy experience.
- Knowledge in intravenous admixture services, intravenous drug therapy and chemotherapy.
- Must be able to work effectively with others to provide safe and effective drug therapy.
- Excellent problem-solving skills and the ability to work independently and quickly in responding to and resolving a wide range of problems.
Job Qualifications:
- BLS Certification.
- ACLS Certification (or shall be obtained upon hire).
- Board of Pharmaceutical Specialties Board Certification.
- Doctor of Pharmacy degree and residency certificate.
- Experience and strong knowledge base in specialty drug therapies.
