Position: COMMUNITY HEALTH EDUCATOR II-CLINIC

Community Health Worker (CHW) II Clinical with previous health education experience.

Under the direction of the department supervisor, the CHW is responsible for assisting individuals with navigating the health and social care systems and providing case management services (under supervision). They act as a bridge between the patient and the medical system by building trusting relationships with community members served by the hospital or program(s).

The position focuses on patients experiencing housing insecurity, particularly those presenting in the Emergency Department and hospital settings across Sinai Chicago. They will navigate patients to housing and community-based resources, coordinating with SUHI’s housing navigation CHWs and clinical teams to ensure continuity of support and connection to services following discharge.

The CHW is stationed in Mount Sinai and Holy Cross Hospitals, working directly with patients providing health education and needs assessments, identifying and addressing patient concerns, screening for social needs, assisting with discharge, and providing resources.

The CHW also helps in identifying barriers to accessing quality care, works with individuals to overcome these barriers, and provides referrals. SUHI is seeking an enthusiastic, passionate CHW candidate who meets job requirements outlined below.

Position Requirements :
  • Must have high school diploma or GED
  • 2 years of health education, community health work, case management, or experience addressing social determinants of health (housing insecurity preferred).
  • Knowledge of Chicago’s West Side and/or South Side communities (resident of West Side or South Side communities preferred)
  • Passion for and dedication to working with community members and providers
  • Punctual, reliable and willing to learn
  • Demonstrated commitment to racial and health equity
  • Must have a valid driver’s license and vehicle
  • Proficiency in Microsoft Word and Excel preferred
  • Bilingual in English and Spanish preferred
