Valid driver's license and clear driving record required. This position includes driving an wheelchair adapted van.
- Provide personal care and assistance with daily living activities for clients.
- Monitor and record patients vital signs and health status.
- Assist with mobility and transfers, ensuring safety and comfort.
- Communicate effectively with clients and their families regarding care plans.
- Maintain a clean and safe environment for clients.
- Document care provided and report changes in client conditions to nursing staff.
- Participate in team meetings and contribute to care strategies.
- Valid CNA certification in the state of Illinois.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team-oriented environment.
- Empathy and compassion for patients and their families.
- Reliable transportation and flexible availability for shifts.
- Willingness to undergo background checks.
- Clean driving record
