SummaryThe Licensed Clinical Social Worker is a key member of the Rush University System for Health community. This position provides in-person and virtual social work services to individuals with complex needs from a variety of referral sources and connects those patients and community members to relevant resources both internal to RUSH and in the community. This role is hybrid with an expectation of providing onsite support to partner sites around Chicago.
This position supports relevant programming focused on supporting individuals experiencing housing insecurity or those who are unhoused and may serve as a representative of their department in meetings and presentations. This position exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.
- Master’s Degree in Social Work.
- Licensed Social Worker (LCSW) in State of Illinois.
- Two years’ experience in healthcare, care management, mental health and/or social services.
- Experience with relevant population(s), including people who are unhoused and psychiatric social work experience.
- Professional qualities such as initiative, self-reflection, commitment, responsiveness and adaptability.
- Experience with high risk, crisis and/or complex populations.
- Strong planning and implementation skills, including problem solving and troubleshooting skills.
- Knowledge of care management, community-based interventions, assessment, counseling Crisis Management and/or motivational interviewing.
- Computer literate (Microsoft Office and others).
- Works directly with patients and their families to address unmet needs and provide the necessary social work services to help them achieve their care goals.
- Carrying out comprehensive social work assessments by phone or in-person to develop a plan of care for the patient and/or family.
- Connects patients and their families to relevant resources, both at Rush and in the community, to address their needs.
- Provides care management and/or support programming for the population as outlined by Manager.
- Documents all client interactions in either the Epic, LCM software and/or other spaces as indicated by Manager.
- Maintains relationships with referring Rush clinics through consistent and timely communication.
- Promotes Rush University System for Health clinical services by referring clients to relevant Rush clinics.
- Represents the department at meetings, conferences or other public engagements on an as‑needed basis.
