The PrEP (Pre-Exposure Prophylaxis) Care Coordinator is responsible for providing care management to a group of patients and assists in prevention efforts to help patients achieve and maintain their optimal level of health. Using a team-based approach, the PrEP care coordinator will act as a resource to patients and health care staff to ensure patients receive all the care and services they require.
This position provides sexual health education, linkage to preventive care services, and supportive services to maintain stable health status to at risk patients. The PrEP Care Coordinator must be proactive, well-organized, present a friendly demeanor and function well in a fast-paced clinical environment while providing the best patient care services. This position is hybrid, with most work completed on site.
Duties and Responsibilities
- Manage an assigned caseload of clients and partners with clinical and supportive team members to ensure the highest quality of care is provided.
- Implement PrEP (Pre-Exposure Prophylaxis) education, care management, and ongoing follow-up to ensure PrEP adherence.
- Provide rapid HIV counseling, testing, and referral services.
- Connect uninsured and underinsured patients with financial support for their medical needs such as patient assistance programs.
- Maintain up-to-date and accurate knowledge of HIV/STI prevention strategies, including PrEP, PEP (Post Exposure Prophylaxis) and related interventions.
- Maintain client file documenting data into the client database and electronic health record (EHR).
- Encourage patients to become actively engaged in their own health through calm, compassionate communication, and by utilizing motivational interviewing techniques.
- Conduct outreach and engagement at community events as needed.
- Submits monthly/quarterly/annual reports in a timely fashion.
- Meaningfully participate in regular administrative and clinical supervision and team meetings.

Activities:
Communicating with care team, reviewing patient charts, identifying gaps in care, identifying areas of need, scheduling appointments, participating in the establishment of the care plan, ensuring fidelity to the care plan. - Other Duties as assigned.

Requirements- Associate’s degree or equivalent work/educational experience in healthcare required.
- Minimum of 2 years’ work experience in health, social, or community services is preferred.
- Current certification as an HIV Counselor or display knowledge of HIV and STI transmission and treatment preferred.
- Ability to speak, read and comprehend English and Spanish fluently.
- Displays strong time management and organizational skills in a high intensity work environment.
- Thrives working in a team-based environment.
- Proficient in Electronic Health Record, Microsoft Word, and Excel
- Strong ability to multi-task and work well in a fast-paced environment.
- Demonstrates problem solving, decision-making, and sound judgment as well as customer service skills.
- Must have flexibility to work on site when required, and work evenings and/or weekends, if necessary.
- Exhibits flexibility to adapt to unforeseen needs or circumstances.
