Job SummaryThe CCHIP Integrated Care Specialist (Specialist) will conduct HIV testing, outreach, recruitment, and linkage to care services, providing enhanced care coordination to persons living with or affected by HIV/AIDS. The Specialist will work closely with newly diagnosed and those vulnerable to HIV, ensuring continuation and expansion of PrEP patient navigation and counseling programs at CCH Ambulatory & Community Health Network Clinics.
The Specialist will promote program enrollment and use of antiretroviral therapy for HIV treatment and prevention. The Specialist conducts retention and case‑finding activities, under supervision, to locate newly diagnosed and lost‑to‑care clients. The Specialist works with a team of peer educators and staff to conduct community outreach, HIV testing, and health education at ACHN locations and throughout the eligible metropolitan area.
This position is grant funded until February 28, 2027.
Typical Duties- Conduct outreach, recruitment, HIV testing, pre/post‑test counseling, and risk reduction counseling on at‑risk populations in clinical settings at assigned ACHN locations.
- Conduct case reporting, partner services, and health outcomes as required.
- Conduct linkage to care, health education/literacy assessment, specialty care referrals and enrollment, and eligibility verification activities for newly diagnosed persons and out‑of‑care persons, including HIV, STI, PrEP, and DoxyPEP education.
- Conduct sessions with individuals newly diagnosed with HIV to link them to EIS services.
- Conduct community case‑finding activities, including outreach and home visits.
- Participate in daily/weekly case conferences and planning with the care team.
- Engage newly diagnosed and/or lost‑to‑care patients in primary care.
- Identify PrEP‑eligible clients and link them to appropriate services.
- Assist in the implementation of PrEP services and appointment scheduling in primary care, ensuring clients receive education about coverage programs.
- Provide individual‑ and relationship‑based HIV and STI counseling.
- Document services rendered in the CCH Electronic Medical Records System (EMR).
- Provide education about Post‑Exposure Prophylaxis (PEP) and other biomedical and prevention interventions; refer eligible clients to primary care at CCH sites.
- Participate in weekly case conferences and planning with the care coordination team in person or by phone.
- Develop a service plan of action to meet client needs, document health education sessions, and complete accurate charts and EMR for Ryan White Chicago Department of Public Health audits.
- Make referrals and linkages to HIV medical care; assist clients with gathering eligibility documentation for Ryan White services (identification, residency, income verification).
- Document attempts, encounters, and services delivered into the EMR as prescribed; maintain related records and logs.
- Enter client encounters and services into CAREWare and EMR under the enhanced care coordination model.
- Under supervision, conduct retention and appointment reminders.
- Make appropriate referrals to prevention‑targeted testing programs; assist with HIV navigation services.
- Adhere to all applicable state, local, and federal law.
- Participate in quality improvement activities.
- Participate in continuing education in HIV to remain current.
- Perform other duties as assigned.
Minimum Qualifications
- High School diploma or GED equivalent with one year of experience in HIV‑related health services (must provide copy of diploma at interview).
- Demonstrated cultural competence and/or experience with diverse populations - MSM, recently released from incarceration, sex workers, extremely low income, substance users, homeless (required).
- Must be able to work non‑traditional hours, including nights or weekends, as requested (required).
