Primo Center is seeking qualified, caring individuals who want to make a difference supporting families experiencing homelessness by working as a Community Home Visiting Case Manager
.

ABOUT PRIMO CENTER

The mission of Primo Center is to empower families to become productive, responsible, and independent members of their community. Today, Primo Center is a leader in providing family shelter and permanent supportive housing, integrated physical, dental, and mental health care, early childhood development, and supportive services to homeless families in Chicago. The organization has considerable capacity and experience as a provider of services to families affected by extreme poverty and homelessness, assisting more than 1100 people each year with the aid of volunteers, over 90 staff members, and an annual operating budget of nearly $10 million.

Primo Center works in high-need communities across Chicago, operating facilities in Austin on Chicago’s west side, Englewood on the south side, and Hermosa on the northwest side.

POSITION RESPONSIBILITIES Provide Intensive Case Management (50%)
  • Facilitating collaboration and communication between different providers and support services to ensure holistic care for the client.
  • Conducting in-depth assessments of the client's situation and specific needs to identify areas for support and intervention.
  • Working closely with the client to set clear, personalized goals and develop a thorough plan of action to actively address their needs and significantly improve their well-being.
  • Assisting the client in identifying and accessing relevant community resources, such as support groups, housing assistance, and vocational services.
  • Maintaining detailed records of services provided and monitoring the client's progress towards their goals.
  • Providing immediate crisis intervention and support during emergency situations to ensure the client's safety and well-being.
  • Provide intensive case management services for families experiencing homelessness and those at risk of becoming homeless.
  • Daily monitoring of family interaction within a communal residential environment, enforce house rules, chores and regulations to ensure a safe and harmonious living space.
  • Collect program specific intake paperwork and conduct intakes for all clients (IM+CANS).
  • Support families in securing benefits, such as enrollment in County Care, Medicaid, TANF, SNAP, WIC, Social Security benefits, etc.
Administration and Client Services Coordination (20%)
  • Maintain ongoing audit ready case files.
  • Maintain client engagement and follow‑up through a variety of means: phone calls, email, visits, etc.
  • Ensure accurate and timely data entry into CIS, IM+ CANS, HMIS, etc.
  • Complete accurate documentation in daily logs, incident reports and any additional reporting requirements.
  • Ensure individuals are supported in the most respectful and professional manner.
  • Ensure that health and safety codes are always adhered to and by all residents.
