Qualified Intellectual Disability Professional; Case Manager

Join Our Team as a Qualified Intellectual Disability Professional (Case Manager) in Chicago, IL!

Are you passionate about making a positive impact in the lives of individuals with intellectual disabilities? Do you have the skills and drive to advocate for and support those in need? Active Visions is seeking a dedicated Qualified Intellectual Disability Professional to provide support to adults receiving CILA services through Active Visions.

• Oversee a caseload of 12-15 individuals to ensure they are safe, healthy and an active participant in their community
• Provide case management services for individuals with intellectual disabilities
• Develop individualized Implementation Strategy Plan based on assessments and evaluations
• Coordinate and monitor services and supports for each individual
• Advocate for clients' rights and preferences
• Collaborate with a multidisciplinary team to ensure comprehensive care
• Provide crisis intervention and problem-solving support
• Maintain accurate and up-to-date documentation
• Supervise Direct Support Professionals working in CILA

• Bachelor's degree in social work, psychology, human services, or related field
• At least one year providing direct services to individuals with intellectual disabilities
• Knowledge of relevant regulations and government benefits
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Passion for advocating for vulnerable populations
