Signify Health is seeking part-time Per Diem Clinicians (NP/PA) to perform in-home health evaluations. Clinicians engage with members in their homes, review medications and history, perform physicals, and coordinate with health plans for appropriate care.
Visits are about 45 minutes, with flexibility to travel within licensed states and conduct virtual visits when needed. Benefits include a base hourly rate plus incentives, malpractice coverage, and supplies.
