Job SummaryIdentify the need for medically related social services and pursue the provision of these services. Responsible for identifying the psycho/social and emotional needs of residents and devising and implementing services/interventions to meet those needs including collaboration with outside community resources.

- Social Work with Nursing Home Experience Preferred
- Education in Social Work or related human services field
- Ability to relate and communicate with diverse populations
- Must have excellent written and verbal communication skills
- Knowledge of community resources
- Exemplify and maintain a high-level of professionalism within an interdisciplinary team
- Experience with geriatric population, dementia care education and experience
- Ability to intervene in crisis
- Knowledge of healthcare setting and terminology
Employees must have the ability to communicate effectively. Requires reading, writing, drawing conclusions from written or computer generated materials. Employees must make assessments and decisions from limited information. Employee must be able to report information, make public contacts and handle multiple priorities. Employee may make non-routine or unexpected judgments and implement recommendations/plans by coordinating persons and/or other resources.
Essential Functions of the Job and Departmental expectations:- Provide support and advocate for resident and/or family.
- Maintain knowledge of community resources.
- Through assessments and care planning, identify and seek ways to support resident’s individual needs and preferences, customary routines, concerns and choices, finding options that best meet the physical and emotional needs of each resident.
- Assist residents with financial and legal matters i.e.:
Advanced Directive completion, applying for Medical Assistance, referrals to lawyers, referrals to funeral homes for preplanning arrangements, referrals to APS. - Identify/provide services/interventions/resources/referrals for grief, bereavement and end of life issues.
- Inform residents, families and responsible party of their rights.
- Participate in discharge planning, development and implementation of social service care plan and multidisciplinary care conference coordination.
- Complete assessments per guidelines.
- Assist in vulnerable adult maltreatment prevention investigation, reporting and employee education.
- Knowledge and on-going education of federal and state regulations as well as facility policies and procedures as they apply to the position.
- Develop and maintain resource directory for Medical Social Worker department.
- Provide employee and community education on Advanced Directives.
- Serve as Hospital Social Worker as needed.
- Provides complementary notary services to residents, families, employees and community.
- Other duties:
