Chronic Disease Management Pharmacist

Responsibilities

• Provide direct face‑to‑face patient care, progressive clinical pharmacy practice; medication optimization supported by a collaborative practice agreement (extension of primary care and/or specialist)
• Manage chronic disease patient panel with a focus on full and shared risk, high‑risk patients with uncontrolled chronic disease states
• Oversee pharmaceutical care, retrieve and evaluate biomedical literature and design medication therapy regimens
• Collaborate with the healthcare team to manage formulary adherence and medication use processes

• PGY1 residency minimum expectation; PGY2 ambulatory or cardio strongly preferred
• Board certification must be obtained within one year of role (BCPS or BCACP, etc)
• Licensed in the state(s) of practice or eligible for reciprocity required
• Immunization and non‑vaccine injectable certification required based on area of practice
• Bachelor’s or Doctorate degree in pharmacy from an ACPE‑accredited school of pharmacy required, or an FPGEC certificate from NABP
