Extended Support Specialist

UCP Seguin is seeking compassionate, dedicated, and dependable professionals to join our team as an Extended Support Specialist (Direct Support Professional - DSP). In this rewarding role, you will provide direct support to adults with developmental and other disabilities, helping them achieve greater independence, enhance their quality of life, and actively participate in their communities. Our mission is to support individuals with disabilities and special needs, enabling them to become productive and valued members of society.

Qualifications
  • High School Diploma or GED required.
  • Must be at least 21 years of age.
  • Minimum of one (1) year of successful work or volunteer experience supporting individuals with developmental disabilities.
  • Valid Illinois Driver's License with an acceptable driving record and insurability approval.
  • Ability to obtain and maintain required certifications, including:
    • Medication Administration Training
    • First Aid and CPR Certification
    • Illinois DSP Training Curriculum Completion
    • Safety Care Behavior Intervention Certification
  • Strong verbal and written communication skills, including proper English grammar and documentation abilities.
  • Basic computer proficiency, including experience with computers, tablets, iPads, and smartphones.
  • Excellent interpersonal skills with the ability to collaborate effectively with consumers, families, coworkers, and interdisciplinary teams.
  • Physical ability to assist individuals with activities of daily living, transfers, mobility assistance, behavioral interventions, and lifting requirements as needed.
Key Responsibilities

The Extended Support Specialist provides support in personal care, daily living activities, transportation, medication assistance, community integration, household management, behavior support, documentation, and advocacy. Employees serve as role models while ensuring the safety, dignity, rights, and well‑being of the individuals they support. This role requires active participation in interdisciplinary teams, implementation of Individual Service Plans (ISPs), and maintaining accurate documentation.

Schedule

This unique position follows a 6-days-on, 7-days-off rotation
, requiring approximately 144 continuous on‑site hours per rotation with guaranteed overtime opportunities and scheduled sleep periods.
