Referral Support Specialist

• Processes and manages referral requests through Erie’s electronic health record (EHR).
• Verifies patient’s financial status and verifies appropriate internal and external referral resources according to Erie’s referral guidelines.
• Ensures all needed information is in the referral through Erie’s electronic health record including patient diagnosis and reason for referral; communicate with providers if any clinical information is missing.
• Verifies patient insurance prior to processing referral using payor platforms.
• Processes referral with the correct hospital contact information and scheduling instructions.
• Sends referrals via Fax or Care Everywhere Referral Management (CERM) to the hospital partner for care coordination.

• Preferred: 1 year of experience in healthcare/medical office or equivalent, a plus.
• Basic computer skills.
• Excellent customer service skills.
• Ability to work independently and within a team.
• Cultural competency to work with a diverse patient population.
• Preferred:
  Proficiency in medical office Spanish a plus.
• Intermediate proficiency in Microsoft Office a plus.
• Intermediate EPIC skills a plus.

Demonstrates proficiency in managing referral processes within electronic health records, ensuring accurate patient information and effective communication with healthcare providers. Capable of verifying insurance and financial status while providing excellent customer service to a diverse patient population.
