Licensed Clinical Social Worker (Admin) - Chicago, Illinois, USA

Under the direction of the Clinical Director, Behavioral Health Services, the Social Worker provides psychosocial services to patients and families of all ages, with emphasis on the emotional and social issues resulting from illness, disability, and/or death. Duties are to be performed in a confidential, professional manner in alignment with the mission and values of Insight Hospital.

Document psychosocial history, initial assessment, interventions, and discharge plans in a comprehensive and timely manner.

Reassess patients upon notification of significant health status changes.

Coordinate with government agencies as needed.

Assist in the development, planning, and facilitation of therapeutic/educational groups for patients and families.

Provide clinical social work services (information, referrals, counseling) to individuals and families.

Ensure compliance with NASW Code of Ethics and hospital mission.

Maintain knowledge of community resources to benefit patients and families.

Provide consultation to healthcare teams on psychosocial management.

Support patient care using Careful Nursing principles: trustworthy collaboration, health education, fostering safety/comfort, participatory management.

Perform other duties as assigned.

Master's Degree in Social Work from an accredited school (Required)

Licensed as a Social Worker in the State of Illinois (Required)

Knowledge of patient growth and development needs

Ability to maintain accuracy in work and cope with stress
