Job Overview

Geode Health is a rapidly growing outpatient mental health provider. We are seeking an Advance Practice Provider (Psychiatric PA or PMHNP) to work in the West Loop market.

• Provide psychiatric healthcare services, including initial mental health evaluations, individualized treatment plans, medication management, and supportive psychotherapy.
• Partner with psychiatrists and mental health therapists to coordinate medication management and psychotherapy.
• Develop and implement treatment plans based on patient needs.
• Maintain accurate electronic health records in the EMR system.

• Active Psychiatric PA or PMHNP license in Illinois.
• Active DEA license.
• Active CDS license.
• Passion for high-quality care with excellent clinical knowledge and communication skills.
• Proficiency in using an EMR system.
• Ability to treat a general spectrum of mental health conditions.
• Minimum of one year of professional work experience in an APP role, preferably from an outpatient setting.
