Do you have a passion for mental health and making a meaningful impact in a long-term care setting? Join our team as a Psychiatric Rehabilitation Services Coordinator (PRSC) and help support residents in achieving their highest level of emotional and social well-being.
Qualifications- Bachelor’s degree in a human services field (e.g., Sociology, Psychology, Special Education, Rehabilitation Counseling).
- Minimum of one year of supervised experience in mental health or human services.
- Knowledge of psychiatric rehabilitation programs and relevant federal and state regulations.
- Strong interpersonal, organizational, and communication skills.
- Compassion, patience, and the ability to work in fast-paced, emotionally demanding environments.
- Builds strong, respectful relationships with residents, families, and staff.
- Maintains composure and professionalism in high-stress situations.
- Approaches challenges with a positive, solutions-focused attitude.
- Thrives in collaborative, interdisciplinary settings.
As a PRSC, you will coordinate, monitor, and support individualized psychiatric rehabilitation programs for an assigned caseload of residents. You will work closely with the interdisciplinary team to ensure emotional and social needs are met with compassion and clinical integrity.
Key Responsibilities Core Functions- Build and maintain a stable therapeutic relationship with each resident.
- Orient new residents to the facility and treatment plan.
- Facilitate resident participation in treatment plan reviews and care conferences.
- Deliver or coordinate psychiatric rehab services tailored to each resident’s needs.
- Monitor residents’ progress in self-directed care and overall treatment plan compliance.
- Assist in developing and managing psychiatric rehab programs.
- Maintain documentation, forms, and reports in compliance with regulations.
- Schedule transportation and coordinate outside appointments as needed.
- Provide consultation and support to facility staff and community partners.
- Participate in staff evaluations and audits to ensure program quality.
- Prepare for and participate in surveys from regulatory agencies.
- Conduct assessments for new and re-admitted residents.
- Lead care plan meetings, write goal statements, and develop discharge plans.
- Offer 1:1 and group interventions focused on self-care, socialization, and community integration.
- Document all resident services and progress in an accurate, timely manner.
- Participate in interdisciplinary team planning and problem-solving.
- Provide resources for financial assistance, discharge planning, and community referrals.
