Continuum Therapy Partners is hiring a Licensed Physical Therapist Assistant (PTA) for a full-time opportunity at Norwood Crossing in Chicago, Illinois
. If you’re looking for a rewarding Physical Therapist Assistant position in a skilled nursing facility (SNF), this is an opportunity to grow your career while helping adult and geriatric patients regain mobility, independence, and confidence.
- Provide skilled physical therapy treatments under the supervision of a licensed Physical Therapist.
- Assist patients with gait training, transfer training, therapeutic exercise, balance activities, strengthening, and functional mobility.
- Monitor patient progress and communicate changes to the supervising Physical Therapist.
- Complete accurate and timely electronic documentation.
- Collaborate with physicians, nursing staff, caregivers, and the interdisciplinary rehabilitation team.
- Educate patients and families on mobility, safety, and home exercise programs.
- Maintain compliance with Medicare, state, federal, and company guidelines.
- Graduate of an accredited Physical Therapist Assistant program.
- Current Illinois Physical Therapist Assistant (PTA) license required.
- Skilled nursing, long-term care, rehabilitation, post-acute care, or outpatient experience is helpful but not required.
- New graduate PTAs are encouraged to apply.
- Strong communication, teamwork, and organizational skills.
