As a Home Health Occupational Therapist, you will help patients regain independence, improve daily functioning, and safely navigate life at home.
Responsibilities- Perform occupational therapy evaluations in patients' homes
- Develop individualized treatment plans based on patient goals and clinical needs
- Provide therapeutic interventions to improve independence, mobility, and daily living skills
- Educate patients and caregivers on safety strategies, adaptive techniques, and the proper use of assistive equipment
- Collaborate with nurses, therapists, physicians, and other interdisciplinary team members to coordinate care
- Accurately complete and submit clinical documentation in a timely manner
- Ensure compliance with agency policies, Medicare regulations, and professional standards of practice
- Active Illinois Occupational Therapist (OT) license
- Current driver's license and reliable transportation
- Home health experience preferred, but not required
- Strong communication and interpersonal skills
- Ability to work independently while effectively managing schedules and patient visits
