As a Home Health Occupational Therapist, you will help patients regain independence, improve daily functioning, and safely navigate life at home.

• Perform occupational therapy evaluations in patients' homes
• Develop individualized treatment plans based on patient goals and clinical needs
• Provide therapeutic interventions to improve independence, mobility, and daily living skills
• Educate patients and caregivers on safety strategies, adaptive techniques, and the proper use of assistive equipment
• Collaborate with nurses, therapists, physicians, and other interdisciplinary team members to coordinate care
• Accurately complete and submit clinical documentation in a timely manner
• Ensure compliance with agency policies, Medicare regulations, and professional standards of practice

• Active Illinois Occupational Therapist (OT) license
• Current driver's license and reliable transportation
• Home health experience preferred, but not required
• Strong communication and interpersonal skills
• Ability to work independently while effectively managing schedules and patient visits
• Dependable, professional, and dedicated to providing patient-centered care
