Home Health Aide (HHA/CNA) – Part-Time / PRN Downtown Chicago & North Shore Suburbs (Skokie, Niles, Wilmette, Winnetka)

One Home Health is seeking compassionate, dependable Home Health Aides and Certified Nursing Assistants (CNAs) to join our growing team. Responsibilities

As a Home Health Aide/CNA, you will:

  • Provide personalized care and assistance in patients' homes
  • Assist with activities of daily living, including bathing, dressing, grooming, and mobility
  • Support patient comfort, safety, and independence
  • Observe and report changes in a patient's condition to the clinical team
  • Assist with basic health-related tasks, including medication reminders and approved personal care services
  • Follow individualized care plans and agency protocols
  • Educate and support family members and caregivers as appropriate
  • Maintain accurate communication with the care team
  • Ensure a safe, clean, and respectful care environment
  • Protect patient privacy and confidentiality at all times
Qualifications

We're looking for caregivers who are passionate about helping others and committed to delivering exceptional care.

Requirements:

  • Previous home health, hospice, private duty, or caregiving experience preferred
  • Compassionate, patient-focused approach to care
  • Reliable, professional, and dependable work ethic
  • Strong interpersonal and communication skills
  • Ability to perform the physical duties associated with patient care
  • Valid driver's license and reliable transportation preferred
