One Home Health is seeking compassionate, dependable Home Health Aides and Certified Nursing Assistants (CNAs) to join our growing team. This Part-Time/PRN opportunity offers flexible scheduling, one-on-one patient care, and the chance to build meaningful relationships while providing exceptional support to patients in their homes.

As a Home Health Aide/CNA, you will:
- Provide personalized care and assistance in patients' homes
- Assist with activities of daily living, including bathing, dressing, grooming, and mobility
- Support patient comfort, safety, and independence
- Observe and report changes in a patient's condition to the clinical team
- Assist with basic health-related tasks, including medication reminders and approved personal care services
- Follow individualized care plans and agency protocols
- Educate and support family members and caregivers as appropriate
- Maintain accurate communication with the care team
- Ensure a safe, clean, and respectful care environment
- Protect patient privacy and confidentiality at all times

Requirements:
- Previous home health, hospice, private duty, or caregiving experience preferred
- Compassionate, patient-focused approach to care
- Reliable, professional, and dependable work ethic
- Strong interpersonal and communication skills
- Ability to perform the physical duties associated with patient care
- Valid driver's license and reliable transportation preferred
