Patient Health Advocate
The Patient Health Advocate I meet individually with HIV impacted patients to provide adherence monitoring and education. Keeps ongoing regular contact with patients throughout their care. Links patients to health/medical and supportive services. Assists patients with resolving their problems or issues. Facilitates and/or coordinates psychosocial support groups.
High school graduate or G.E.D. equivalent. Excellent communication, organizational and writing skills. Basic Microsoft Word, Excel and Power point skills. Prior health/medical and/or social services experience a plus. Experience servicing individuals with substance abuse and/or mental health. At least 2 years of experience serving individuals impacted by HIV/AIDS.
Responsibilities- Assist in ensuring patient adherence to medical and referral appointments through reminder phone calls, sending out referral correspondence.
- Complete Data Entry for Evidence Based Project Partnership for Health as required by Center for Disease Control and Prevention.
- Collect data from medical records including STI prevention messages, occurrence of STI's and medical visits.
- Submit required monthly data for Partnership for Health project to supervisor.
- Complete program paperwork with patient including intake of new patients, nutritional screen, adherence questionnaire, referrals, and ADAP paperwork as needed.
- Conduct adherence monitoring, post-test HIV counseling and education with patients as needed.
- Meet with patients individually during their medical appointments in order to reinforce prevention and education messages as well as assist with strategies for medical compliance.
- Assists patients with transportation arrangements to medical services as needed.
- Assist in the coordination of Consumer Advisory Board (CAB) meetings and activities.
- Facilitate and/or coordinate psychosocial support groups.
- Facilitate patient referral process including follow up and tracking.
- Assist with community outreach activities, such as health fairs, community agency meetings, as requested.
- Maintain appropriate SOAP documentation of services provided in patient medical charts, referral binder, etc.
- Participate in multidisciplinary staffings and patient consultation sessions with medical provider and other key clinical and supportive staff as needed.
- Complete monthly reports, case reports, quality activity reports and other reports as requested.
- Provides individual and group level HIV/AIDS/STI education to patients.
- Coordinates and links patient to medical and supportive services, i.e. referrals for dental, mental health/substance abuse, nutrition, support groups, case management, patient assistance, etc.
- Maintains working relationships with HIV Team members and services to enhance coordination.
- Attend all program related meetings and identified HIV/AIDS/STI and other related trainings as scheduled.
