• Provide counseling services at Austin Health Center with caseloadsstructured and managed to support effective, patient-centered care.
• Complete an individualized treatment plan for each patient oncaseload, addressing all needs identified in the psychosocial assessment.
• Provide group and individual sessions to patients in accordance with their treatment plan problems, goals, and objectives.
• Review and evaluate patients’ progress and continued stay in treatment in relation to measurable goals and objectives described in treatment and care plans. Modify treatment plans to comply withchanges in patient status.
• Plan and assist with the implementation of aftercare and continuingrecovery plans once patients are discharged from treatment programs.
• Maintain a minimum of 50% of time in direct, face-to-facecounseling contact.

Pay Rate:$43,000 – $ 55,000 per year; basedon certification and experience

Education and/or

Experience:

High School Diploma or equivalent required. Bachelor’s degree (or higher) and prior substance abuse treatment experience preferred. Will consider relevantinternship experience in lieu of paid employment.

Certificates, Licenses, Registrations: Must possessor be eligible to obtain IAODAPCA Certified Alcohol and Other Drug Counselor (CADC) certification. Individuals not currently certifiedare required to obtain the CADC within two years of hire or transferinto the position, in accordance with applicable experience and qualification requirements. Certification must be maintained in good standing as a condition of continued employment.
