OverviewPosition Summary: The Medical Case Manager provides comprehensive, patient-centered, and confidential services to individuals living with HIV/AIDS, facilitating access to core medical and support services. These include medical, dental, mental health, and substance use services, as well as supportive resources such as legal services, transportation, emergency financial assistance, food access, and housing support. The primary objective is to promote and sustain viral suppression by helping patients remove barriers to care to achieve and maintain an undetectable viral load.
Responsibilities- Oversee and manage an assigned caseload of patients living with HIV/AIDS, providing comprehensive Medical Case Management services.
- Complete intake into care, including comprehensive initial and ongoing assessments of patient strengths and needs, including HIV medical status, insurance coverage, financial status, adherence to appointments and medications, behavioral health and substance use, oral health, nutrition, transportation, and other psychosocial needs.
- Maintain knowledge of internal and community resources, develop a referral database, submit referrals, and provide follow-up to ensure patients are connected to appropriate services and remain engaged in care.
- Develop comprehensive care plans and monitor patient progress toward meeting collaboratively developed goals of care.
- Maintain patient files, both physical and digital, in accordance with the standards established by Prime Care Community Health (Prime Care) and the AIDS Foundation of Chicago (AFC).
- Collaborate with the medical and interdisciplinary team, including medical providers, hospitals, and insurance companies, to provide integrative care to patients with HIV.
- May participate in outreach services, including presentations, trainings, and development projects, as needed.
- Effective, concise verbal and written communication with a diverse set of professionals from multiple disciplines.
- Ability to thoroughly assess and analyze materials, especially application forms and related documents.
- Highly detail-oriented with strong attention to deadlines and timelines.
- Respect, appreciation, inclusion, and sensitivity for the cultural diversity, gender differences, and sexual orientation of patients and co-workers.
- Ability to work flexible and extended hours and travel between sites and/or community-based locations, as needed.
- Demonstrated knowledge and at least six (6) years of work experience equivalent to one of the following education credentials (degree not required if work experience meets the same level):
- Associate’s degree in human services, social science or health education plus four (4) years of case management experience
- Bachelor’s degree in social work, human services, social science, or health education
- Master’s degree in social work, human services, social science, or health education
- Current Illinois licensure as Licensed Practical Nurse (LPN), Registered Nurse (RN), or Advanced Practice Registered Nurse (APRN)
- Prior experience working with individuals living with HIV/AIDS strongly preferred
- 3-5 years of experience in a healthcare or medical setting, preferably in a Federally Qualified Health Center
- Working knowledge of Electronic Health Records (EHR) and MS Office applications
- Bilingual English/Spanish preferred, but not required
- Eligibility for NHSC Loan Repayment or State Loan Repayment programs
- PTO: 27 days per year, accrued each pay period
- 3 personal days

All wages are based on relevant years of experience and/or licensure. The minimum rate is the wage that an individual who has no relevant experience earns. No individual will begin employment earning more than the maximum wage listed, which is expected to occur with 13 or more years of relevant experience.
