We are seeking a dedicated and compassionate Social Services Assistant to join our interdisciplinary team full-time. In this role, you will help support our residents' psychosocial, emotional, and social needs to ensure they receive holistic, high-quality care throughout their stay.
Responsibilities- Assist in identifying residents' psychosocial, mental, and emotional needs.
- Complete social histories and psychosocial assessments for new and current residents.
- Collaborate closely with the interdisciplinary clinical team to advocate for and protect residents' rights.
- Help maintain a safe, respectful environment by actively preventing and addressing resident concerns or grievances in accordance with facility policies and mandated reporting guidelines.
- Assist residents and their families with transition planning, community resources, and daily care adjustments.
- Complete required documentation accurately within the Electronic Medical Record (EMR) system.
- Perform other related administrative and supportive duties as assigned by the Director of Social Services or Facility Administrator.
Education:
- LSW / CSW Social work license in the state of Illinois
Specific Skills/Experience REQUIRED:
- Experience in long-term care, discharge planning, care planning, resident advocacy, and knowledge of state and federal regulations.
Skills/Experience PREFERRED:
- Experience working in a skilled nursing facility, strong communication and organizational skills, and experience in working with residents, families, and interdisciplinary teams.
Technical & Compliance Requirements:
- Proficiency with computers and Electronic Medical Record (EMR) systems.
- Strong customer service background with a proven track record of positive resident, family, and community relations.
- Must pass all mandatory Illinois state healthcare worker background check requirements.

- Excellent communication and interpersonal skills with the ability to build rapport quickly with residents, families, and team members.
- Strong organizational skills and the ability to work effectively both independently and as part of a collaborative team.
