Position Summary
The Community Care Connection (CCC) Part-Time Family Nurse Practitioner (FNP) or Physician Assistant (PA) is responsible for providing high quality, patient-centered medical care and health education through the Community Care Connection mobile clinic (Care Coach) as part of the Rosalind Franklin University Health Clinic (RFUHC). This role supports the delivery of accessible healthcare services to underserved and vulnerable populations within the community.
The Family Nurse Practitioner or Physician Assistant will serve a vital role in advancing access to healthcare for underserved and vulnerable populations through the Community Care Connection mobile health program while also functioning as an integral member of the RFUHC Primary Care team. This position provides comprehensive primary and preventive healthcare services across the lifespan, including health assessments, diagnosis and treatment of acute and chronic conditions, health screenings, patient education, and care coordination.
Working collaboratively within an interdisciplinary healthcare team, the APP will help support the delivery of a Patient-Centered Medical Home (PCMH) model of care, promoting quality outcomes, continuity of care, disease prevention, and population health management. The successful candidate will demonstrate a commitment to health equity, community engagement, and evidence-based practice while contributing to the educational mission of Rosalind Franklin University.

- Travel throughout Lake County to community-based service locations with the Community Care Connection (CCC) Mobile Screening and Education Unit, providing clinical services approximately three to five days per week as scheduled.
- Maintain flexibility to support community healthcare initiatives, including occasional evening clinics (5:00 p.m.
- 8:00 p.m.) and up to two weekend outreach or screening events per month. - Provide comprehensive primary and preventive healthcare services, including health assessments, wellness screenings, patient education, and follow-up care in accordance with established clinical standards, evidence-based practices, and organizational policies.
- Conduct patient evaluations, obtain and document comprehensive medical histories, perform physical assessments, and develop appropriate treatment and care plans within the scope of practice.
- Accurately document patient encounters, diagnoses, treatment plans, and evaluation and management (E&M) coding within the Athena Electronic Health Record (EHR) system.
- Monitor, assess, and document patient conditions, screening results, and outcomes, communicating findings and recommendations to patients and other healthcare team members as appropriate.
- Provide patient and family education regarding disease prevention, health promotion, medications, nutrition, wellness strategies, and recommended follow-up care.
