Position Overview
Under general supervision, performs professional social work services in the treatment of mentally ill patients on an assigned unit. These services include (among others) assessment, treatment, writing/submitting court reports, performing general case management, discharge planning and where indicated acting as a qualified examiner. Ensures compliance with Joint Commission, Central Management Services (CMS) and other agency standards and objectives. Serves on the Department and/or Hospital committees as necessary at Chicago-Read Mental Health Center. Utilizes American Sign Language with patients/individuals who are deaf and/or hard of hearing.

Essential Functions
  • Performs professional social work services in the treatment of mentally ill individuals on an assigned unit.
  • Provides case management (includes completing funding applications, psychoeducation for consumer and family, acting as liaison between hospital and community agencies advocate for the consumer, etc.), individual therapy, family therapy and group therapies as directed by the master multi- disciplinary treatment plan for patients under his/her direct care and for patients on his/her unit in the case of group therapies.
  • Has a principal role and responsibility in the discharge planning process.
  • Assumes responsibility to act as qualified examiner as necessary for evaluating, making clinical assessments, completing certificate and testifying in court.
  • Completes all documentation of assessments, treatment and discharge planning according to hospital policy and mental health code requirements.
  • Serves on the Department and/or Hospital committees as necessary at Chicago-Read Mental Health Center.
  • Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
Conditions of Employment
  • Requires the ability to restrain patients as necessary to prevent injury to patient and others.
  • Requires the ability to obtain and maintain Cardiopulmonary Resuscitation (CPR) certificate.
  • Requires the ability to stoop, bend, lift or exert up to 20 pounds of force in performance of duties.
  • Requires the ability to physically access various worksites throughout the hospital for administrative and clinical duties.
  • Requires the ability to testify in court.
  • Requires the ability to travel in the performance of job duties.
  • Requires ability to pass the IDHS background check.
  • Requires ability to pass a drug screen for drugs prohibited from recreational use under Illinois Law.

Masters, 1+ Years work experience
Minimum Qualifications
  • Requires a master’s degree in social work from a recognized college or university supplemented by one (1) year of professional social work experience in the evaluation and treatment/habilitation of mentally ill, developmentally disabled or other disabled persons.
  • Requires the ability to communicate through American sign language at an Intermediate Plus skill level per the Sign Language Proficiency Interview (SLPI).
