Hopedale Medical Complex is seeking a compassionate Physical Therapist to provide one-on-one, evidence-based rehabilitation services across our hospital, clinic, and senior living settings. You will evaluate patients, develop and implement individualized treatment plans, deliver therapeutic exercises and modalities, and collaborate closely with physicians, nurses, and other therapists. This role is ideal for a therapist who values community-based care, strong relationships with patients and families, and contributing to a close-knit, rural healthcare team focused on functional independence and quality of life.

Responsibilities
  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans
  • Deliver therapeutic exercises, manual therapy, and modalities to improve function and mobility
  • Provide gait, balance, and fall-prevention training for adult and geriatric patients
  • Educate patients and families on home exercise programs, safety, and recovery expectations
  • Document assessments, progress notes, and outcomes accurately in the EMRCollaborate with physicians, nursing, and interdisciplinary team members on care plans
  • Adapt interventions across hospital, clinic, and senior living environments
  • Participate in care conferences and discharge planning to support safe transitions
  • Monitor patient progress and adjust treatment plans based on clinical response
  • Contribute to a compassionate, community-focused culture and quality improvement efforts
