Basic Function
Under the supervision of a Licensed Practitioner of the Healing Arts (LPHA), the Crisis Continuum of Care Community Based Clinician (CBC) provides community-based services for 90 days, community based behavioral health assessment, crisis intervention, stabilization, safety planning, care coordination, and linkage to ongoing behavioral health services for children, adolescents, adults, and families experiencing behavioral health crises.
Basic Function
Under the supervision of a Licensed Practitioner of the Healing Arts (LPHA), the Crisis Continuum of Care Community Based Clinician (CBC) provides community-based services for 90 days, community based behavioral health assessment, crisis intervention, stabilization, safety planning, care coordination, and linkage to ongoing behavioral health services for children, adolescents, adults, and families experiencing behavioral health crises.
The Crisis Continuum of Care Community Based Clinician serves within Ada S. McKinley's integrated Crisis Continuum of Care Program, delivering trauma-informed, recovery-oriented, culturally responsive, and person-centered behavioral health services in homes, schools, hospitals, emergency departments, community settings, and other approved locations.
The Crisis Continuum of Care integrates Mobile Response and Stabilization Services (MRSS), the Crisis Stabilization Program (CSP), and the 590 Crisis Program into one coordinated system designed to provide immediate crisis response, stabilization, diversion from unnecessary emergency department utilization and psychiatric hospitalization, and timely linkage to ongoing behavioral health treatment.
Principal Duties And Responsibilities
- Performs comprehensive, in-depth assessment and clinical formulation, development of accurate diagnosis and strengths-based treatment plans for Crisis Continuum of Care services.
- Provides a full range of case management, counseling, advocacy, and educational services.
- Identifies and utilizes internal agency and external community resources to meet client needs.
- Assesses broader community conditions impacting clients to identify systemic issues and resource gaps.
- Provides group-based services as indicated by program needs.
- May be responsible for assisting with 24-hour crisis coverage.
- Represents and interprets agency programs to the community, fostering collaboration and partnership.
- Supports and attends community events, including initiatives to increase safety and community cohesion.
- Establishes and maintains a strong referral network with outside agencies to ensure continuity of care for participants and their families.
- Takes a proactive role in the development and enhancement of services and programming that integrate trauma-informed care principles.
- Meets agency expectations for quantity and quality of direct service delivery.
- Completes all required agency documentation and record-keeping in a timely and accurate manner.
- Works effectively and respectfully with a diverse staff and participant population.
- Other duties as assigned.
Program Flexibility
- Provide services within MRSS, Crisis Stabilization Program, and the 590 Crisis Program.
- Maintain competency across all Crisis Continuum of Care programs.
- Complete all required program-specific training.
- Adapt to changing assignments based on operational needs.
Collaboration
Collaborate with Team Leads, Program Manager, Director, LPHAs, Peer Support Specialists, Case Managers, Nurses, APNs, Psychiatrists, hospitals, schools, law enforcement, 988, community providers, and Managed Care Organizations.
Documentation & Reporting
- Maintain timely and accurate electronic health record documentation.
