Senior Care Coordinator, Therapeutic Services

Job Summary

In this role, the Senior Care Coordinator, Therapeutic Services is responsible for delivering quality, patient-centered care for individuals seeking mental health/therapeutic services. They will oversee a caseload of up to 30 community members. This is in collaboration with and under the supervision of the Assistant Care Manager, Therapeutic Services. This candidate will be part of the Supportive Services team. This position is grant funded.

• Hold a caseload of up to 30 mental health clients, providing 1:1 counseling services, completing notes, etc.
• Follow fidelity monitoring protocols in data collection and documentation in Apricot.
• Develop and implement engagement resources for mental health services.
• Provide drop in mental health services.
• Apply research principles and relevant subject matter knowledge to administer a research project. With a moderate level of direction, manage lab and/or research-related duties and tasks. Help develop, design and conduct research projects according to plan.
• Support data collection and analytical needs of research projects. Conduct literature reviews and help write reports and manuscripts. Ensure project compliance with different policies, procedures, directives, and mandates.
• Perform other related work as needed.

Minimum qualifications include a college or university degree in a related field. (Required)

Minimum requirements include knowledge and skills developed through 2‑5 years of work experience in a related job discipline.

• Advanced degree.
• Clinical License (LCPC, LCSW, etc.).
• Experience working with and/or providing care to LGBT populations, Black and Latino community members.
• Experience providing services for people with HIV and/or HCV.
• Proficiency in Microsoft Applications, including but not limited to Outlook, Excel, Access and Word.
• Effective leadership and teaching skills.
• Organizational skills.
• Knowledge of Chicago social service resources.
• Knowledge of program subject matter.
• Interpersonal skills and the ability to work both independently and as part of a team.
• Flexibility.
• Creativity.
• Ability to manage stressful situations.
• Ability to maintain confidentiality.
• Ability to work on multiple projects simultaneously, set priorities, and meet deadlines.
• Ability and/or willingness to provide HIV/STI treatment, education, and prevention in non‑traditional settings.

• Ability to work on multiple projects.
