BHT​/CASE MANAGER

Talbot Health Services – Main Street Residential Program

Talbot Health Services is looking for a dependable, experienced Residential Case Manager/BHT to work in our Main Street residential program in Toledo.

This is a hands-on position for someone who understands substance use disorder treatment and can work effectively with residents in a structured setting. The person hired will help maintain the daily operation of the residence, support residents through treatment, coordinate practical services, and communicate closely with the clinical and medical teams.

We are especially interested in applicants who hold an Ohio CDCA or higher credential. Applicants without a CDCA may be considered if they have meaningful, directly related experience working in residential addiction treatment, behavioral health, recovery services, or case management.

• Work directly with residents throughout the residential program.
• Maintain a safe, organized, and respectful living environment.
• Provide structure, accountability, encouragement, and practical support.
• Complete case management work related to appointments, transportation, identification, benefits, housing, employment, continuing care, and community resources.
• Help residents follow their treatment and recovery plans.
• Observe and report changes in behavior, mood, functioning, or safety.
• Communicate resident needs and concerns to counselors, nurses, providers, and supervisors.
• Complete required progress notes, shift notes, incident reports, and other documentation accurately and on time.
• Support residential routines, schedules, house expectations, groups, appointments, and daily responsibilities.
• Follow Talbot Health Services policies and all assigned safety, crisis-response, confidentiality, and medication-related procedures.
• Participate in team meetings, case reviews, training, and discharge-planning activities.
• Help residents transition successfully to continuing care and community-based supports.

• Has experience working with people affected by addiction and co-occurring mental health conditions.
• Understands that residential treatment requires consistent presence, follow-through, boundaries, and accountability.
• Can remain calm and professional when residents are upset, resistant, or experiencing a crisis.
• Is reliable, observant, organized, and able to document what occurs during a shift.
• Can build trust without losing professional boundaries.
• Works well with counselors, nurses, providers, supervisors, and residential staff.
• Holds an Ohio CDCA, CDCA Preliminary, or higher credential, or has comparable direct experience in addiction or residential behavioral health services.
• Can work the schedule required by a residential program, including assigned evenings, weekends, and holidays.
• This position is a good fit for someone who wants to be actively involved in residents’ recovery and understands the responsibility that comes with working in a residential treatment setting.
