Essential Duties and Responsibilities:
1. Deliver Compassionate Patient Care:
Perform comprehensive assessments, including history-taking, physical exams, and diagnostic evaluations.
Develop and implement individualized treatment plans that address acute and chronic conditions.
Provide preventive care and health education to empower patients in their health journey.
2. Collaborate with a Multidisciplinary Team:
Partner with physicians, nurses, behavioral health specialists, dental providers, and other team members to deliver holistic care.
Participate in case reviews, care planning, and team meetings to support patient wellness and continuity of care.
3. Engage with the Community:
Participate in outreach initiatives and health promotion activities to improve access to care.
Build trust and relationships with patients, families, and community partners.
4. Focus on Quality and
Innovation:
Maintain accurate, timely documentation in our electronic health record (EHR) system.
Support quality improvement initiatives and leverage data to enhance care delivery.
5. Champion Equity and Inclusion:
Deliver culturally responsive care that reflects the diverse needs of the communities we serve.
Commit to reducing health disparities and improving health equity.
Qualifications:

Required Education/Licensure/
Experience:
Master’s degree in Physician Assistant Studies from an accredited program.
Current licensure as a Physician Assistant in Illinois.
Certification by the National Commission on Certification of Physician Assistants (NCCPA).Current DEA license (or ability to obtain).Minimum of 2-3 years of clinical experience, preferably in primary care, internal medicine, or community health.
Experience working with underserved or diverse populations is highly desirable.
Skills and
Competencies:
Strong clinical decision-making and communication skills.
Ability to thrive in a collaborative, fast-paced, and mission-driven environment.
Proficiency with electronic health records (EHR) systems (Athena Health preferred).Bilingual skills (e.g., Spanish) are a plus but not required.

Pay Transparency:
The salary range for this position is $100,000 - $135,000 annually. Actual compensation will be determined based on factors including, but not limited to, relevant experience, qualifications, internal equity, and organizational needs.
