The Referral Coordinator is responsible for managing the referral intake process from hospitals, home health agencies, and other healthcare referral sources. This role reviews incoming referrals and supporting documentation for completeness, determines whether referrals meet company requirements, and coordinates timely follow-up and processing of accepted referrals.
The ideal candidate is highly organized, detail-oriented, and comfortable communicating with hospitals, healthcare professionals, referral partners, patients, and internal teams. The Referral Coordinator plays an important role in ensuring referrals are handled accurately, promptly, and efficiently while maintaining complete and organized records throughout the referral process.
- Monitor and receive incoming patient referrals from hospitals, home health agencies, and other referral sources.
- Review referral information and supporting documents for completeness and accuracy.
- Check whether referrals meet the company’s requirements and determine whether they can be accepted or should be declined.
- Follow up with hospitals and referral sources regarding missing or additional information.
- Communicate with referral sources promptly once a referral is received.
- Coordinate accepted referrals with the appropriate internal team members.
- Maintain accurate records and documentation of all referrals.
- Track the status of referrals from receipt through acceptance, follow-up, and processing.
- Ensure referrals are followed up on in a timely manner to avoid delays.
- Answer incoming calls and assist with referral-related inquiries when needed.
- Communicate professionally with hospital staff, referral partners, patients, and internal team members.
- Assist with other administrative and intake-related tasks as needed.
- Previous experience in healthcare administration, referral intake, home health, medical office, or a similar role preferred.
- Strong attention to detail and ability to review information carefully.
- Excellent communication and follow-up skills.
- Comfortable speaking with hospitals, referral sources, and healthcare professionals.
- Strong organizational and time-management skills.
- Ability to handle multiple referrals and tasks simultaneously.
- Comfortable learning company-specific referral criteria and processes.
