The Lead Diabetes Educator reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The Lead Diabetes Educator provides excellence in diabetes self-management education and support services and promotes increased awareness of diabetes in our service area. This position is the quality coordinator for Diabetes Self-Management Education and Support Services (DSMES) program.
Responsibilities- Utilizes appropriate patient care protocol relating to the physical and psychological needs of the pediatric, adolescent, adult, gestational, and geriatric patients. Communicates with physician/order provider and documents appropriately in the medical record.
- Provides care for both outpatient and inpatient patients with diabetes.
- Is the quality coordinator of the DSMES program. Oversees the planning, implementation and evaluation of the DSMES program. Responsible for annual program meeting with annual program review and program plan.
- Serves as chair of the diabetes advisory committee.
- Responsible for overseeing daily billing of diabetes clients.
- Designs, evaluates and implements diabetes self-management education and support services based on individual, physician and community needs in accordance with American Diabetes Association guidelines.
- Coordinates orientation and continuing education for staff specific to the care needs of individuals with diabetes.
- Completes and compiles records, data, policies and procedures required by the department, hospital and outside agencies.
- Participates in the preparation of the diabetes education budget and selecting performance review criteria.
- Generates, collects, enters and analyzes data and uses it in decision‑making and reporting. Represents the department and needs of diabetes education program at assigned committees.
- Maintains professional growth through literature review, seminars, and membership in organizations related to diabetes education.
- Facilitates provision of information on hospital issues to team.
Required:
- Bachelor’s degree in dietetics or related field
- Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
- Registered Dietitian (RD) by Commission on Dietetic Registration or Registered Nurse. State licensure required by state law. Certification by the National Certification Board of Diabetes Educators (CDE).
