About the RoleAnuco Home Health Care LLC is seeking compassionate, dependable, and qualified Home Health Aides to provide personal care and supportive services to patients in their homes.
The Home Health Aide will follow an individualized plan of care under the direction and supervision of an RN or assigned therapist. The ideal candidate is respectful, reliable, attentive to changes in patient condition, comfortable traveling to patient homes, and committed to timely documentation and accurate electronic visit verification.
Responsibilities- Provide personal care services authorized in the patient’s plan of care.
- Assist with bathing, grooming, dressing, oral hygiene, toileting, and other activities of daily living.
- Assist patients with safe ambulation, transfers, positioning, and assigned exercises.
- Assist with medications that are ordinarily self-administered only when authorized and permitted.
- Perform simple procedures as an extension of nursing or therapy services after receiving proper instruction and demonstrating competency.
- Provide essential household services related to the patient’s health and safety when included in the plan of care.
- Observe the patient’s physical, emotional, and environmental condition during every visit.
- Promptly report changes in condition, falls, injuries, skin concerns, pain, medication concerns, hospitalizations, or unmet needs to the supervising clinician.
- Report accidents, incidents, complaints, suspected abuse or neglect, and unusual occurrences immediately.
- Follow infection-control procedures, standard precautions, hand-hygiene requirements, and equipment-cleaning procedures.
- Protect patient privacy and confidentiality and comply with HIPAA.
- Treat patients, caregivers, their homes, and personal property with dignity and respect.
- Attend required staff meetings, competency evaluations, in-services, and continuing education.
- Complete all duties assigned by the supervising RN or therapist.
- Decline and report any assignment requiring skills for which the aide has not been trained or determined competent.
- Clock in and out accurately for every visit using the agency’s approved EVV system, including HHAeXchange when assigned.
- Complete visit documentation accurately in Axxess or the designated agency system within required time frames.
- Document all personal care and supportive services provided during the visit.
- Ensure recorded arrival and departure times match the actual visit.
- Obtain required patient or caregiver verification when applicable.
- Report missed visits, late arrivals, early departures, schedule changes, and technical problems immediately.
- Notify the office before performing services that are not listed in the plan of care.
- Respond promptly to documentation corrections and administrative requests.
- Understand that inaccurate, incomplete, or late documentation may delay care coordination, billing, payroll processing, and future assignments.
- At least 18 years of age.
- High school diploma, GED, or equivalent preferred.
- Current Illinois CNA certification or other qualification that satisfies applicable Home Health Aide requirements.
- Home health or direct patient-care experience preferred.
- Current CPR or BLS certification.
