This is a full-time, on-site opportunity located in Chicago, IL. Applicants must live in Chicago or a nearby surrounding area. The role includes a transportation stipend.
Why This Role MattersThe HUB Case Manager is a highly visible, relationship-driven role that combines nursing expertise, workers' compensation knowledge, employee education, and operational support. This position plays a key role in helping employees successfully navigate their recovery and return-to-work journey while supporting leaders and stakeholders throughout the process.
Unlike traditional clinical nursing roles, the HUB Case Manager serves as a trusted advisor, educator, and case coordinator. The role offers the opportunity to build meaningful relationships with employees, provide guidance during challenging situations, and positively impact both employee outcomes and operational success.

Key Responsibilities
- Coordinate return-to-work and transitional duty programs for employees recovering from occupational injuries or illnesses.
- Provide employee outreach, education, and support regarding recovery, workers' compensation processes, return-to-work expectations, and available resources.
- Review work restrictions and identify suitable modified-duty opportunities in partnership with operational leaders and stakeholders.
- Serve as a subject matter expert on workers' compensation, disability management, return-to-work programs, and employee work restrictions.
- Maintain ongoing communication with employees and stakeholders to address barriers and facilitate successful return-to-work outcomes.
- Track employee absences, case milestones, and return-to-work activities while maintaining accurate documentation across multiple systems.
- Manage a caseload and prepare reports, analyses, and recommendations for leadership regarding employee restrictions, absences, and program effectiveness.
Qualifications
- Education: Associates Degree or Bachelor’s Degree in Nursing or related field.
- Experience:
- 2+ years of clinical, case management, disability management, absence management, workers' compensation, occupational health, or return-to-work experience preferred.
- Experience coordinating return-to-work programs, managing work restrictions, transitional duty assignments, employee absences, or workers' compensation cases strongly preferred.
