Axiom Care is seeking an experienced MDS Director to join their team in Flora, IL. The successful candidate will provide MDS guidance, ensure compliance with regulations, and optimize reimbursement processes for skilled nursing facilities. Ideal applicants will have a strong background in MDS with at least 3 years of experience, possess strong audit and training skills, and hold an active RN license (BSN preferred).

This role includes collaboration with facility teams to enhance practices and support survey readiness.
