About the Role

We’re building a world of health around every individual — shaping a more connected, convenient and compassionate health experience. As a Manager, Care Strategy and Operations at Oak Street Health®, a part of CVS Health, you will own the strategy and execution for the Diabetes and Hypertension programs. You will lead a small team while translating program design into consistent, measurable delivery across Oak Street centers nationwide.

Responsibilities
  • Manage the program portfolio and lead strategy, execution, and continuous improvement for Diabetes and Hypertension programs.
  • Evaluate how these initiatives interact with broader Oak Street projects to drive clinical and economic results.
  • Build and scale the full lifecycle of clinical interventions—from pilot design and launch through proof of concept to national scale.
  • Lead a team of 1–3 direct reports, building capacity while maintaining accountability for program outcomes.
  • Drive cross‑functional alignment with clinical, operational, and regional leaders to build business cases and advance initiatives without formal authority.
  • Collaborate with regional teams to translate central strategy into consistent clinical and operational practice at the center level.
  • Monitor and communicate key metrics—quality measures, utilization, financial impact—and use data to inform program decisions.
Qualifications
  • 5+ years in healthcare strategy, consulting, population health, or clinical operations.
  • Bachelor’s degree required.
  • Proven ability to launch pilots and scale initiatives across a distributed, national footprint.
  • Strong project management skills in complex, multi‑stakeholder environments.
  • Experience managing or mentoring others with a collaborative leadership style.
  • Comfortable synthesizing data to build compelling business cases and influence senior stakeholders.
  • Thrives in ambiguous, high‑growth environments; change‑oriented without losing rigor.
Benefits and Compensation
  • Mission‑focused career impacting change and measurably improving health outcomes for Medicare patients.
  • Paid vacation, sick time, and 401(k) match options.
  • Health, vision, and dental insurance.
  • Opportunities for leadership development and continuing education stipends.
  • New centers and flexible work environments.
  • Opportunities for high levels of responsibility and rapid advancement.
  • Typical pay range: $66,330.00 – $, with potential bonuses, commissions, or short‑term incentive programs.

Oak Street Health is an equal opportunity employer. We embrace diversity and encourage all interested applicants to apply.
